Leave of Absence LESS Than 30 Days
Employee receives Medical Leave of Absence packet from the Main Office or from their Local Committeeman.
Employee completes Medical Leave of Absence Form 32006 and submits to Car Shop Management for approval.
Employee completes Railroad Retirement Board Statement of Sickness Form and Aetna (formerly Broadspire) Notice of Disability Form. Local Committeeman or Occupational Health Nurse will assist, if needed.
It is critical that the employee completes required forms within 7 days. If not filed within 7 day period, you may lose benefits.
Employee obtains a release to return to work from their physician and submit it to the Car Shop Management.
Car Shop Management authorizes employee’s return to work.
Employee returns to work.
Leave of Absence GREATER Than 30 Days
Employee receives Medical Leave of Absence packet from the Occupational Health Nurse, Main Office, or the Local Committeeman.
Employee confers with Occupational Health Nurse.
Medical Progress Report Form 16920 or Railroad Retirement Board Statement of Sickness Form is completed by employee’s medical provider and returned to Occupational Health Nurse.
Medical Leave of Absence Form 32006 is completed and submitted to Car Shop Management for approval.

Occupational Health Nurse approves the employee’s return to work. Car Shop Management authorizes employee’s return to work.
Employee returns to work.
